Pharmacy Coordinator Rx Recon

The Pharmacy Coordinator Rx Recon will coordinate and assist clinical pharmacist to perform medication reconciliation post hospital discharge to reduce hospital re-admission rates, hospital bed-days, ER visits and total healthcare costs as well as maintain an atmosphere of professionalism and confidentiality.



Job Responsibilities/Duties

  • Telephonically schedule patient appointments for medication reconciliation consultations. 
  • Process referrals for medication reconciliation. 
  • Communicate telephonically, via fax, and or email to obtain pertinent information for consultation from patients, physician offices and case managers. 
  • Assist patients and prescribers with any barriers in obtaining medications.
  • Provide support to clinical pharmacists as part of disease management processes.
  • Utilize designated company software to document required information such as demographic information and pertinent medication current medications. 
  • Produce necessary reports on a regular basis, using Microsoft Excel to prioritize patients and maintain a consistent workflow.
  • Reconcile provider responses to medication reconciliation recommendations on a daily basis and clarify ambiguous or unclear responses as needed.
  • Perform light clerical duty such as formatting and printing patient letters.


Qualifications

Minimum Education: High School Diploma required.

Minimum Experience: None.

Req. Certification/Licensure: Active pharmacy technician license required. PTCB/national pharmacy technician license preferred.
