Disease Management Program (DMP) Coordinator

This position has accountability for supporting DMP functions and care team to maximize program efficiency and effectiveness. Tasks include but are not limited to the following: identifying/tracking ESRD patients through appropriate channels, process referrals and follow up on prospective DMP/ESRD patients; maintain/track Protected Health Information (PHI) of identified high risk members; use of and data input to medical management electronic systems; manage provider and member program materials; and complete reports and other deliverables as assigned.



Job Responsibilities/Duties

  • The DMP/ESRD coordinator will identify ESRD patients from registry list for identification and transition through appropriate process according to the company's policies and procedures.
  • Will work with other members of Medical Management to complete ESRD patient transition as appropriate.
  • Will reconcile all members identified monthly or as needed to ensure completion of process.
  • The DMP/ESRD Coordinator will process incoming referrals and follow up on prospective DMP patients identified through PMS high risk lists and/or transition of care (TOC). DMP Coordinator ensures that referrals are addressed in a timely manner according to set standards.
  • DMP identification and referrals come from claims data-mining, Transition in Care Program, Case Managers (CM), Medical Directors, Pharmacists and contracted providers. DMP Coordinator will notify referral sources of unsuccessful outreach of referred patients.
  • The DM/ESRD Coordinator conducts physician office outreach, requests copies of patient's medical records. and provides education on the PMS DMP/ESRD process, and shares specific patient information on identification for the program as requested.
  • The DMP/ESRD Coordinator conducts member outreach to invite and encourage DMP participation and schedule appointments with DMP providers.
  • DMP Coordinator administers the appropriate baseline quality performance metric for the program and alerts DMP care team and/or CM as appropriate.
  • The DMP Coordinator enters information and medical records in organization's care management platform and other systems in accordance with standardized protocols.


Qualifications

Minimum Education: High school graduate required; Associate in Arts degree preferred.

Minimum Experience: Experience in healthcare. Experience in effective outreach and engagement.
