Medical Reconciliation RN

The main responsibilities of the the Medical Reconciliation RN include: performing medication reconciliation for patients discharged from an inpatient facility; coordinating with internal and external healthcare professionals for transitions of care; providing support & collaborating with pharmacists for member care.



Job Responsibilities/Duties

  • Provide medication reconciliation for members discharged from an inpatient setting by comparing the pre-admission and discharge medication lists. Conduct medication reviews to identify and resolve issues with appropriate documentation in a central case management system. Assist members to be compliant with medication regimens, arrange copay or financial assistance programs as appropriate, and provide education. Consult with physicians to resolve drug-related issues.
  • Coordinate with other Medical Management’s functional teams (e.g., Case Management, Disease Management) to provide transitions of care for discharged and/or high-risk members including follow up touches as needed.
  • Collaborate with other healthcare professionals to provide clinical care for members managed by pharmacists through different clinical programs (e.g., disease management programs, interdisciplinary team for Model of Care). 


Qualifications

Minimum Education: None. 

Minimum Experience: A candidate with transitions of care and/or case management experience is preferred. 

Req. Certification/Licensure: The candidate must hold an active RN license.
